Home Health Physical Therapist (PRN)

AdvisaCare Home Health is currently seeking a PRN Home Health Physical Therapist to join our team! As a Home Health Physical Therapist, you will have the opportunity to provide individualized care to patients in the comfort of their own homes. Responsibilities

• Perform evaluations, establish goals and develop treatment plans for patients based on individual needs and diagnosis
• Assess and document patients' progress towards achieving their goals
• Provide education to patients and their families about home exercise programs and proper body mechanics
• Coordinate care with other healthcare professionals involved in the patient's treatment
• Maintain accurate, thorough and timely documentation of patient care and progress
• Participate in interdisciplinary team meetings and provide input on patient care
• Collaborate with other healthcare professionals to ensure continuity of care

Requirements

• Current Physical Therapy license in the state of Michigan
• Minimum of one year of experience as a Physical Therapist with home health care experience is preferred, but not required
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively
• Valid driver's license and reliable vehicle
