DaVita IKC is seeking a dedicated and knowledgeable IKC RN Expert with extensive complex case management experience to step into a pivotal role on our Specialty Care team.
This is more than a case management position; it's an opportunity to become a subject matter expert and a central point of contact between our IKC and Specialty Care teams. You will be the strategic facilitator for our Specialty Care teams, responsible for reviewing and triaging complex patient escalations and ensuring a seamless transition of care for our most vulnerable patients. You must excel at clinical assessment, communication, and process improvement skills in this role to allow you to make a significant impact across the Village.
This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. This position provides coordination of care between the patient, family, physician, provider and care teams, the community, and Specialty Operations Manager.
NOTE: The DaVita IKC position is remote telephonic-based and will require a home office.

Qualifications for the Shining Star for our DaVita IKC Registered Nurse are:
· A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degree
· Current Registered Nurse (RN) license in the state of practice required
· Continuing education credits maintained as required by state of practice required
· Minimum of five (5) years’ experience in clinical nursing required
· Minimum of three (3) years’ experience in renal nursing preferred
· Significant experience within DaVita's Integrated Kidney Care (IKC) model is strongly preferred.
· Proven expertise in clinical chart review, patient assessment, and care planning.
· Exceptional communication skills, with the ability to provide clear, consultative feedback and guidance to clinical peers.
· Proficiency in Cerner and a strong understanding of IKC workflows and escalation processes.
· A strategic thinker who can identify trends, solve problems, and work collaboratively to improve processes.
