Abode Home Health is currently seeking a full time Physical Therapist - Doctor of Physical Therapy to support our skilled nursing patients in Pueblo, CO.  The home health PT will make intermittent home visits to patients in a particular area.  Enjoy the flexibility that home healthcare can bring and apply today!

Responsibilities

• Provide physical therapy services to patients according to a written physician’s plan of care
• Initiate physical therapy program and instruct other personnel and/or family/caregiver members as appropriate
• Prepare and submit clinical and progress summaries
• Provide in-service education programs for nursing organization personnel as needed
• Participate in discharge planning
• Supervise Physical Therapy Assistants according to policy and state regulations
• Ensure compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Qualifications

• Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in state of operation
• 2 years of experience in adult physical therapy
• Home health experience preferred
• Commitment to clinical and documentation excellence
