But it requires tailored, timely, intelligent patient engagement throughout the patient’s lifelong care journey.

Lumata takes a holistic approach to care—combining behavioral and psychosocial information with clinical data to create tailored, high-impact touchpoints. In addition to promoting compliance, we help patients with things like financial assistance programs, prescriptions and refills, insurance issues, transportation to visits, understanding their disease and care plan, and coordinating care with other providers.

Position Overview

As the mVT Specialist, you will ensure patients enrolled in the myVisionTrack® (mVT) program are effectively onboarded and supported, facilitating improved patient outcomes through regular vision monitoring. Your daily work will involve calling patients and onboarding them to the mVT mobile app over the phone, monitoring patients' mVT app usage, following up with patients to help them maintain physician-recommended testing adherence through the app, and completing necessary documentation to support accurate reporting.

Schedule

  • 8 hour shift
  • Monday to Friday


Pay: $19.00 - $23.00 per hour

Essential Functions:

Patient Onboarding and Education:

  • Call patients who are eligible for mVT and help them install and use the app to monitor their vision. This will involve making app installation calls to patients that are handed off to you by our Care Coaches, in addition to your own outreach to a list of patients enrolled in our program.
  • Meet monthly patient onboarding goals.

Patient Support and Engagement:

  • Follow up (via phone and/or text) with patients who are non-adherent with testing through the app. Achieve 90% patient adherence to physician-recommended testing frequency.

Coordination with Healthcare Providers:

  • Ensure seamless communication between patients, Care Coaches, and healthcare providers.

Technical Support and Troubleshooting:

  • Resolve 95% of basic technical issues independently and escalate complex problems appropriately.

Training and Compliance:

  • Complete required mVT training videos.
  • Create and maintain internal mVT-specific training documentation.
  • Stay up-to-date with app changes and updates. Communicate changes to Care Coach team as necessary.

Feedback and Improvement

  • Collect mVT program feedback from patients and providers and collaborate on at least two improvement initiatives quarterly.

Requirements

Qualifications:

  • Education: COA certification preferred (but not required).
  • Experience: Minimum of 2 years in a patient-facing role within a healthcare setting, with a focus on patient education and support. Experience in Ophthalmology is preferred.
  • Technical Skills: Proficiency in using mobile applications and digital health tools. Ability to walk patients through the app download process over the phone. Basic troubleshooting skills for technical support.
  • Communication: Excellent verbal and written communication skills.
  • Interpersonal Skills: Strong interpersonal skills with a patient-centric approach.
