The Mary T Family of Companies is hiring for a full-time Hospice Clinical Manager to work from our office based in Coon Rapids, MN.
The Clinical Manager is responsible for creating an environment of patient-focused care, coordination, and management of the professional nursing care for the patients. The Clinical Manager helps to ensure the consistent application of policies through management and training of nursing staff. In addition, the Clinical Manager is responsible for ensuring compliance with comprehensive regulations established by the Community Health Accreditation Partner (CHAP) and is responsible for compliance with Medicare conditions of participation.
ESSENTIAL FUNCTIONS AND BASIC JOB DUTIES
- Provide a comprehensive assessment of the health status of a patient through the collection, analysis and synthesis of data used to establish a health status baseline and plan of care, and address changes in a patient’s condition.
- Collaborate with the health care and individual support teams to develop and coordinate an integrated plan of care.
- Develop nursing interventions to be integrated with the plan of care.
- Implement nursing care through the execution of independent nursing interventions.
- Implement interventions that are delegated, ordered, or prescribed by a licensed health care provider.
- Delegate nursing tasks or assigning nursing activities to implement the plan of care.
- Provide safe and effective nursing care.
- Provide a safe and therapeutic environment.
- Advocate for the best interests of individual patients.
- Evaluate responses to interventions and the effectiveness of the plan of care.
- Collaborate and coordinate with other health care professionals in the management and implementation of care within and across settings and communities.
- Participate in the development of health care policies, procedures, and systems.
- Ensure the incorporation of new information and methods into practice.
- Attend meetings and participate in internal organizational activities.
- Manage and monitor nursing staff performance and skill levels, maintaining documentation as required for performance issues, and completing performance reviews as directed by the Director of Hospice.
- Oversee staff training; identify training needs to improve skills and to be following rules and regulations governing hospice.
- Triage incoming calls during business hours
- Facilitate the IDG team Meetings
QUALITY IMPROVEMENT
- Provide leadership through establishing professional and clinical performance goals.
- Participate in quarterly management meetings.
- Provide input in schedule assignments and review schedules to ensure complex patients are assigned to staff competent to provide complex care.
- Participate in ongoing quality assurance activities.
PATIENT SAFETY/SATISFACTION
- Assist Director of Hospice with monitoring currency of individual renewals and annual training.
- Ensure Hospice Aide training is current and supervisory visits are completed per agency policy.
- Provide direct field staff supervision of new hires.
- Perform State of Care, Discharges, Recertifications, and Skilled Nursing Visits.
- Ensure patients and families are satisfied with hospice services.
- Serve as a patient advocate.
- Serve as a resource person to nurses, aides, social workers, Chaplain, and auxiliary team members for questions regarding policy/regulatory issues.
- Keep Director of Hospice informed of any significant problems.
WORK DIRECTION AND/OR SUPERVISION DUTIES
Responsible for providing direction and supervision for all nursing and patient care staff.
BASIC QUALIFICATIONS FOR THIS POSITION
EDUCATION:
- Bachelor’s Degree Nursing preferred; Associates Degree in Nursing required.
CERTIFICATION/LICENSURE:
- Valid RN license in good standing, MN Driver’s License
EXPERIENCE:
- Minimum two (2) years of experience working as an RN in Home Care, Hospice and/or Care Management preferred.
- One (1) year(s) or more in a supervisory position required.
KNOWLEDGE, SKILLS, ABILITIES:
- Ability to manage in varied disciplines
- Knowledge of and ability to practice and demonstrate safe patient handling and proper body techniques related to lifting, pushing, pulling, etc. for all physical activities
