In absence of Director or when necessary, assists the Director to plan, direct, supervise, and monitor the activities of the department to achieve optimum patient care by meeting established goals and objectives. Coordinates these activities with the Medical Staff, Administration, families, ancillary services, and community agencies to enhance total patient care.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. Valid driver’s license.

3. CPR certification within 30 days of hire date.

EXPERIENCE:

1. Three (3) years of progressive Registered Nurse experience.

2. Two (2) years of supervisory experience.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelors of Science in Nursing (BSN).

EXPERIENCE:

1. Two (2) years of previous experience in a hospital/home care/hospice leadership role preferred (such as charge nurse, or documentable consistent demonstration of strong leadership skills while functioning as a staff nurse).

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an exhaustive list of all responsibilities and duties.  Other duties may be assigned.

1. Maintain working relationships with patients, families, staff, LPNs, physicians, and referral sources.

2. Take phone calls and make appropriate documentation of these calls.

3. Obtain orders from physicians as needed.

4. Assists with critical lab reporting/monitoring.

5. Performs and monitors basic patient care according to policies and procedures.

6. Implementing, and evaluating to meet the patient’s physical and psychological needs.

7. Assumes on-call responsibility per WVU Medicine home care policy/procedure.

8. Performs essential functions of the RN.

9. Communicates patient care problems or potential problems to the Administrative staff/director/Nurse of Home Health/Hospice as identified/occur to assure continuity and coordination of care.

10. Documents patient status and care or services furnished in appropriate areas, as defined by the documentation policy and procedure.

11. Monitor supervisory reports for LPH/HHA Compliance.

12. Performs CPR when directed by patients/appropriate representative.

13. Displays safe, efficient, and prudent use and maintenance of DME and medical supplies and adaptive equipment.

14. Performs and monitors patient teaching according to the policy and procedure.

15. Coordinates care so that medications are administered as ordered by a physician and in compliance with the Medication Administration Policy and Procedure.

16, Assists in orienting new staff to the role as needed.

17. Exercises independent professional judgment as evidences by identifying potential problems and taking appropriate action.

18. Monitors faxed labs.  Contacts physician offices of abnormal labs.

19. Supervises staff schedule as requested, resolving any problems and keeping orders current. Maintains schedules to assure low to adequate coverage, clear patterns, and adequate notice to Associates to achieve outcomes of department, and oversees productivity.

20. Assists with duties of the director when out of the office.

21. Completes Discharge/Transfer DIC work queues.

22. Maintains working knowledge of flow-charting, EPIC, Kronos, Word, and Windows, per department needs.

23. Assists director with Kronos compliance

24. Review discharge oasis assessments for quality and monitor deficiency ques as indicated.

25. Monitor SOC forms for charting compliance.

26. Completes monthly record of potentially avoidable events and relate findings to director.
