Job Description:

This position serves a critical function in the strategic execution and physician practice advancement of HPMG/PriMed’s Epic Community Connect Program, HillChart.  The role will adhere to defined processes working alongside operational system users, completing work within a discipline or specialty area.

Specialization

Under the leadership of the HillChart Program Director, the HillChart Health Information Management (HIM) Analyst’s primary role is to ensure Data Integrity of the patient record by performing oversight of medical records in the capturing and entering of data elements required for charting, coding, billing and state/federal reporting, and addressing deficiencies of incomplete records.  As a member of the HillChart Program Team, the HIM Specialist will promote proper physician practice and organizational processes and workflows that fall under HIM; chart oversight, deficiency tracking, release of information, transcription, data conversion, scanning, confidentiality and privacy. They will work closely with Program Director to assists practices with their HillChart needs and alignment with HPMG strategic initiatives, while maintaining strong physician practice relationships and overall program satisfaction.

  • Performs health information quality analysis, collection and management duties needed to organize, maintain and use patient medical records in EPIC
  • They will perform Chart Correction tasks in the EMR, including but not limited to resolving patient duplicates and overlays. They will have understanding of all the data elements required and analyze records for completeness and electronically assigns deficiencies to clinicians in the EMR
  • Ensures and maintains the data integrity and quality of the EMR and Enterprise Master Patient Index (EMPI) and oversees security and safety of all patient information
  • Perform physician liaison and will be responsible for working effectively with the practice staff, health care providers and other personnel as needed to expedite the ongoing and timely completion of documentation in patient records for compliance with regulatory, accreditation, and/or related standards, policies or practices for medical records
  • Duties also include effective and timely follow-up with providers regarding priority chart completion requirements including both pre-suspension and delinquent charts
  • Individuals must work well independently and effectively in identifying methods for reviewing documentation timely and effectively following up to obtain information and/or prompt timely completion of documentation that may be needed in patient charts

Job Responsibilities:

  • Generally performs single function tasks that tend to be of  limited size and scope
  • Uses existing procedures to identify and solve routine or • standard problems
  • Analyzes information and standard practices to make judgments
  • Responds promptly to the needs of end-users, and meets Service Level Agreements as defined in conjunction with our customers following issues through to closure.
  • Participates in weekly team meetings to discuss team and project related activities, issues, change, communications, and updates.
  • No supervisory responsibility
  • Accountable for developing technical skills

Experience

  • Must have at least one (1) years' experience with Epic Community Connect
  • Experience with the Epic system applications highly desirable.
  • Experience with project management fundamentals a plus
  • Demonstrated ability to exercise good judgment and prioritize multiple
