Overview

Chronic Care Staffing is seeking a compassionate and organized Remote Enrollment Coordinator - CMA/RMA to support patients enrolling in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. In this fully remote role, you will serve as a trusted guide for patients, helping them understand available healthcare support services while ensuring all enrollment and documentation processes remain compliant with CMS and HIPAA standards.

This opportunity is ideal for Certified Medical Assistants who enjoy patient interaction, healthcare education, and remote clinical coordination in a fast-paced environment.

Responsibilities

  • Contact patients to introduce CCM and RPM services and explain program benefits.
  • Guide patients through the enrollment and consent process.
  • Document all patient interactions and enrollment activities accurately within EMR systems.
  • Ensure compliance with CMS, HIPAA, and internal documentation standards.
  • Answer patient questions regarding care coordination services and program expectations.
  • Collaborate with healthcare providers and internal teams to maintain smooth communication and patient transitions.
  • Maintain organized workflows while managing daily outreach goals and follow-up tasks.
  • Support positive patient experiences through empathy, professionalism, and clear communication.

Requirements

Must Have

  • Active, nationally recognized CMA or RMA certification.
  • Current Basic Life Support (BLS) certification.
  • Proven experience working directly with CCM and/or RPM programs.
  • Strong proficiency with EMR systems and Google Suite.
  • Dedicated, HIPAA-compliant home office environment that is private and distraction-free.
  • Strong time management, organizational, and independent work skills.
  • Excellent verbal and written communication abilities with strong patient empathy.

Nice to Have

  • Previous experience in care coordination or transitional care management.
  • Strong understanding of CMS regulations, healthcare documentation, and quality reporting standards.
  • Bilingual fluency in English and Spanish.
  • Experience providing health education to patients and families.
