Job Role Summary
Under the direction of a licensed clinical staff member, the CMA assists the providers of a health care site with the facilitation of client visits and procedures, routine laboratory tests, administering immunizations, medication delivery, and referrals. The CMA will work together with the providers to achieve quality outcomes for the population of the practice site. This position will provide follow-up and outreach for individuals within the community and assist with Directly Observed Therapy of clients receiving curative treatment for tuberculosis disease, contacts of active tuberculosis clients receiving preventive or window period treatment for latent tuberculosis and other clients as deemed appropriate. There will be travelling in Marion County.
Essential Duties
- Performs appropriate procedures, treatment regimens (including administration of medications in clinic) administer vaccines and immunizations and delivers medications according to physician orders and/or MCPHD protocols and policies. This includes obtaining vital signs (blood pressure, heart rate, respiratory rate, temperature, height, weight, O2 stat, vision screening), blood draws for various testing, and performing point of care testing in clinics (Rapid HIV, Rapid Syphilis health check, Urine pregnancy test, Urine dipstick).
- Helps provider with providing education (under the direction of a licensed clinical staff member), referrals for needed services.
- Provides TB testing, home visits, and can administer medication and follow-up (primarily directly observed therapy).
- Trained to take medical history, clinically can get prior authorizations.
- Collects demographic information, health history, patient knowledge and beliefs, and extent of illness or health issues for follow-up and continuity of care; scans/index documents as appropriate; answer calls and emails regarding civil surgeon visits as needed.
- Delivers targeted, culturally appropriate information to help individuals and/or families to understand health promotion and disease prevention information, policies, regulations, and local code; evaluate outcomes of direct patient care and education.
- Monitor clients for adverse reactions to medication and/or treatment and report to medical personnel in charge.
- Monitor adherence to therapy; medical appointments; recommendations for isolation; positive and negative motivational factors influencing adherence; and any unmet educational needs of the patient.
- Participates in the coordination of care to individuals and families who are members of a vulnerable population and/or high-risk groups. Collaborates with MCPHD departments, co-workers, resettlement agencies, Indiana State Department of health (ISDH), and other community agencies to provide necessary services.
- Collaborates in and contributes to individual, team, and departmental performance management, quality improvement and evaluation activities.
- Under the direction of licensed medical care provider, assures labs and/or diagnostic services are completed and/or notifies client’s that they are needed. Examples include but are not limited to Chest X-ray, follow-up on abnormal labs, blood draws, and CLIA waived tests.
- Maintain working files of clinic patients. Documents client/patient assessment and intervention data in medical records. Data entry is clear, concise, accurate, and recorded in a timely manner. Uses established medical record forms, databases, and documentation practices.
- Adheres to ethical principles, confidentiality, and MCPHD policy in collection, maintenance, use, and dissemination of data and information.
- Participates in TB case finding, contact investigation, and case conferences.
- Participates in periodic team, staff, and committee meetings.
- Assumes responsibility for own professional growth and development by pursing education; Attends in-services/seminars to remain current in profession.
