This is a per diem part-time position, 3 days a week, with an occasional Saturday 4-hour shift. No holidays!
Responsibilities:
- Working with Kendal-Crosslands Communities primary physicians within a Collaborative Practice Agreement to provide medical on- site support for residents in the Community.
- Identifying and managing chronic and acute health conditions by requesting laboratory and/or x-ray studies, initiating treatment and/or managing medication regimens with appropriate follow up.
- Administering first aid to Residents by evaluating injuries and arranging further treatment if necessary.
- Participating in planning for residents’ needs as they transition from levels of care. Example: Independent Living to Personal Care to Skilled Care.
- Collaborating with the Director of Resident Care on any anticipated or present problems.
- Performing medical procedures as necessary.
- Adhering to Pharmacy protocols and procedures. NP must be comfortable e-prescribing and using computer systems.
- Administering treatments, injections, and medications to residents as prescribed, and as necessary.
- Communicating abnormal lab and tests results to ordering physicians and residents.
- Managing INR Lab results and follow up when necessary.
- Providing wellness screenings and responding to Emergency situations. Following up of other assessments, in person or by phone as appropriate.
- Responding to requests from residents, KCC primary care physicians, consultant physicians and the health care team.
- Initiating requests for consultations or referrals.
- Participating in Quality Assurance, Pharmacy and Infection Control committees as requested by the Director.
- Communicating efficiently with the Resident Care Team.
- Arranging and establishing orders for transfer to the Health Center or hospital, assisting in discharges from hospital to KCC Health Centers.
- Interviewing and evaluating prospective KCC residents to determine appropriateness for admission to the Community.
Requirements:
- Master's degree in Nursing
- Licensed as an RN in the Commonwealth PA
- Certified as a NP in the Commonwealth of PA
- Acute and/or long-term care experience is preferred
- One year recent experience as an Acute Care, Adult-Gero, Gero or Family Practice Nurse Practitioner Setting
- Ability to obtain a current Drug Enforcement Agency registration. Current Prescriptive Authority approval by the Commonwealth of PA
- Ability to work collaboratively with physicians/healthcare providers and relate/communicate well with older people
- Strong leadership and communication skills.
